Need a refill of your current hormone or peptide therapy? Submit a refill request and we'll review your chart and let you know if anything further is needed.


To help us process your refill quickly, please include:
 

• The medication or peptide you are requesting
• Your current dose and how often you take it
• How much you have remaining
• Any changes in symptoms, side effects, or concerns since your last visit
• Any recent labs or relevant updates we should know about
